Not all families stay with Sojourner Home forever, but Sojourner Home stays with them, providing ongoing case management and support services for as long as they need it.
Home away from home
Most families come to Sojourner Home following crisis and trauma, and they work with case managers to develop plans to address those situations. As families build stability, the staff at Sojourner Home works with them to adjust goals, elevate plans and continue to work toward self-sufficiency.
Families who begin their journey in Sojourner Home’s transitional housing programs benefit from a seamless transition to Sojourner Home’s supportive housing programs. Some families secure permanent housing elsewhere in the community but stay connected with Sojourner Home through outreach services, receiving continued case management and other supports from Sojourner Home staff.
